Adivive (Adult-Derived Stem Cell Fat Grafting) Frequently Asked Questions
In the summer of 2012, Dr. Lewis Obi was asked to contribute his thoughts on Adivive and adult-derived stem cell grafting to the leading Australian beauty magazine, Cosmetic Surgery and Beauty Magazine. He was asked several questions about his use of the Adivive system and his typical patient results. The following are those questions, including frequently asked questions by patients.
The Adivive Fat Processing Unit is the most advance stem cell concentrated fat grafting system available. Not only is the system FDA-approved but it is the only totally self contained unit that processes adult fat into a stem cell concentrated filler useful for both cosmetic and reconstructive application.
Palomar sent Dr. Obi the first FDA-approved system in January of 2011 and he has performed approximately 100 fat grafting procedures with this system since January of 2011. Prior to integration of the Adivive technology Dr. Obi had performed various fat grafting techniques for more than 25 years.

The Adivive Fat Processing Unit squeezes and filters harvested fat to provide a cleaner and more concentrated fat of extraordinary quality. The resulting fat graft contains a higher concentration of adult stem cells, cytokines and other growth factors which enhances the long term persistence of the Adivive fat. Also, larger volumes may be safely used. As a result, more dramatic and predictable results occur with an average take in-excess of 50% of the transferred Adivive fat.
The two most commonly treated areas are the face and breasts. To a lesser degree, Dr. Lewis Obi has applied this technology to treating hands, buttocks and areas of the body with deficiency resulting from trauma or previous surgery.
The most common donor sites for large volume fat harvesting has been the abdomen and thighs as these donor sites are simultaneously treated with the SlimLipo laser platform. Adivive for breast enhancing has been most useful for smaller volume grafting (100-150cc per breast). Better results are seen in patients requiring perhaps one cup size enlargements - such as lumpectomy/irradiation patients, breast lift patients and for cosmetic patients with moderate asymmetry of the breasts. It is also much less invasive then the autologous flaps and therefore with much less downtime and fewer risks. The contralateral breast size/sag may be effectively treated with the SlimLipo laser thereby eliminating the need for any surgery. Even after implant reconstructions, Dr. Lewis Obi has used Adivive fat grafting to finalize symmetry and to enhance nipple-areolar reconstruction. In the augmentation patient, moderate enlargements may be accomplished as well as non surgical revisions of breast asymmetry. Adivive may also be used to enhance breast flap thickness in the mastectomy patient. Alloderm™ (cadaver skin) and Strattice™ (pig skin) is universally used by plastic surgeons to provide better coverage of breast implants and to enhance breast folds in these mastectomy patients. Unfortunately, there is a geometric increase in medical costs as well as increased patient risks when these foreign skin grafts are utilized.
For both breast reconstruction and cosmetic breast procedures, Dr. Obi generally utilizes 200 cc or more of processed fat. This may require removal of 500 cc or more of fat from the donor site. In order to avoid contour irregularities of the donor site, SlimLipo is almost always used to treat these areas. The only exception to this rule is if the same area will be revisited for future fat harvesting. Therefore, the total time for treating the breasts and donor sites is approximately 2 to 3 hours.
There are many benefits fat transfer for the face. To begin, Dr. Obi almost always combines small volume (40cc or less) Adivive fat grafting with almost every surgical face lifting procedure. Larger volume fat grafting to the face may be utilized as a totally non-surgical technique for three dimensional tightening of the face. This is a procedure that Dr. Mark Berman of L.A. refers to as a “3-D Space Lift” and more recently, Dr. Lewis Obi coined the term “The OPERA Lift”. Not only does OPERA stand for: Obi Palomar External Rejuvenating Aesthetics, but a patient may undergo the procedure on a Monday and may be presentable enough to attend the Opera on Saturday. The facial tightening is accomplished with large volume Adivive fat and the neck tightening with the SlimLipo laser.
Visual results, especially in the face, are immediate. There is of course some bruising and swelling which resolves in seven to 10 days. Optimal results are seen at different intervals depending on the areas treated and patient variability. Three months seems to be the magic number for both optimal results and prediction of viability of the grafted fat. According to Drs. He Young Lee (Seoul, Korea) and Kotaro Yoshimura (Tokyo), "What you see, in terms of persistence, at three months, we should see long term. That has been my experience during the almost 30 years I have utilized fat as a graft."

This may then be injected into facial areas such as the cheek creases, the marionette creases, lips, cheekbone areas and chin. In addition to the Adivive facial rejuvenation, neck tightening with the LifeSculpt (SlimLipo) system may also be performed at the same time. Both procedures are generally performed under local anesthesia with a preoperative sedation taken orally as an option. The majority of patients may have these procedures performed on a Friday and return to work on the following Monday. The fee for Adivive facial fat grafting performed in this manner is $3500. If LifeSculpt laser neck tightening is performed at the same time then the total fee would be $4900. It should be noted that currently, comprehensive Adivive Fat Grafting facial procedures are performed under nurse sedation because larger quantities of fat is harvested, processed and then injected into more diffused facial areas. This technique also results in a larger fee and more recovery time.

Top Five Non Surgical & Minimally Invasive Alternatives to Popular Plastic Surgery Procedures

minimally invasive options that may restore their youthful appearance.
Through the use of Botox, a patient may smooth forehead creases while
lifting their brow line. Adivive fat grafting utilizes unwanted fat from the
body and refines it to a durable large volume filler for the face. And finally,
the LJO Brow Lift is a minimally invasive proprietary brow lifting procedure
that can be performed in half an hour with little-to-no residual scarring.

2. Liposuction and Body Sculpting
Certain areas of fat may not always respond to exercise and dieting.
Traditional techniques for the removal of this fat are associated with long
periods of recovery, general anesthesia, greater risks and less than optimal
results. However, with the introduction of SlimLipo laser lipolysis four years
ago, fat removal and skin tightening has never been so easy. An added benefit
is that skin tightening continues for several months so that any subsequent
weight loss is synergistic. One of SlimLipo’s multiple benefits is that patients
immediately see visible results following their procedure and continue to see
volume loss and skin tightening up to six months. During the actual

4. Non Surgical Nose Job (Rhinoplasty)
Whether it’s from trauma or ethnicity, some patients have deficient nasal bridges or dorsal humps that may be unsightly. With the use of particular
fillers, such as Radiesse or Coapatite, these deficiencies may be corrected non
surgically. It’s extremely beneficial to patients that have been considering
permanent cosmetic surgery but aren’t sure if it’s the right choice. This

VALENTINE’S DAY WEEK HERALDS AN ERA OF RENEWAL
During the past four years, Dr. Lewis Obi has been one of the lead American plastic surgeons to introduce, develop and teach new and effective, minimally invasive procedures. “Plastic Surgery in the New Age” has been the title and theme of his international symposia in 2012. Slim Lipo, Adivive and PRP are but a few of the newer non invasive procedures he helped pioneer.
This week’s new review will explore not only the most popular current plastic surgery procedures but also look into the top countries for plastic surgery. Although the USA is tops in plastic surgery in terms of sheer numbers, South Korea surpasses us when it comes to per capita numbers. Cosmetic surgery is also popular in both Greece and Italy where 10 procedures were performed for every 1,000 people in 2011.
Ironically, Valentine’s Day this year occurs on the second day of Lent and two days after “Fat Tuesday”. Therefore our theme “An Era of Renewal” proposes some of the minimally invasive procedures exemplified below. Finding Harmonious Balance and Symmetry with Cosmetic Surgery
Being born with a perfect body or facial symmetry is very rare - in fact it’s
very unusual to have identical body halves. One of the hallmarks of striking
beauty is the presence of symmetry.

For most people, the imbalance is virtually undetectable, but sometimes it
may be a little more noticeable. One breast may be a cup size larger, the nose
may be deviated and even an eyebrow may be higher than its counterpart.
When the balance becomes more than just a minor issue, corrective surgery
may be necessary to attain optimal symmetry.
Body Asymmetry
Breast Balance
One of the most common issues women report with their breasts are
asymmetrical cup sizes. An extreme example is a rare situation that Dr. Lewis
Obi encountered several years ago:
In most cases, a patient may have a breast that is a complete cup size larger
than the other. Typically, to correct this imbalance, a plastic surgeon will use
breast implants and/or other techniques. For patients with moderate
asymmetries who do not wish to receive implants, there are other viable
options including Adivive fat transfer and SlimLipo Laser tightening/reduction. With the Adivive procedure, fat is harvested from an area of
undesirable fat. The fat is then processed by the totally enclosed Adivive
system with centrifuges and filters to develop a purer and more viable fat
graft. This purer form of fat is then added to the breasts to restore breast
symmetry and performed without incisions nor sutures. The opposite breast
may then be reduced and or tightened with the SlimLipo Laser
platform...again non-surgically.

Glute Balance
Similar to the breasts, the glutes may be asymmetrical also. Although it may
not be visible to the casual eye, someone with a more critical eye may notice
the dissimilarities. Again, the combination of Adivive fat grafting to one side
with the possibility of SlimLipo tightening are non surgical tools to enhance
the buttocks.
Stomach, Waist and Hips
As we age, fat may deposit in lower areas of the body and become
increasingly difficult to burn with diet and exercise alone. It may be
necessary to remove fat via SlimLipo laser lipolysis in order to balance out
the waist, stomach and thighs. Most commonly, women opt to receive laser
liposuction and breast implants at the same time in order to not appear too top
or bottom heavy after they’ve healed.
Facial Asymmetry
Uneven Eyebrows
Whether it’s a nerve or muscle malfunction, uneven eyebrows are common
among men and women alike. In the past, plastic surgeons have performed
brow lifts if the asymmetry is way off but most recently, it’s been discovered
that Botox can aide in the correction of the raised brow. Dr. Lewis Obi has a
proprietary brow lift procedure he’s been performing for over 30 years called
the LJO Brow Lift that also can assist in the correction of the raised or
droopy brow to create a more symmetrical appearance.
Cheeks and Lips
The facial skeleton and the overlying soft tissues of the face are uneven in
99% of people but usually not noticed except by close observation. However
when the patient becomes aware of the asymmetry, they often consult with a
plastic surgeon. Minor asymmetries are generally not treated but with the
advent of non invasive treatments such as fillers, Botox, Adivive, etc. then
more of these patients are helped.
Nose and Chin Balance
The ideal profile balance for the face would be a less prominent nose and a
more prominent chin. Whether the imbalance occurred from birth or trauma,
it can be corrected – surgically and non surgically. Chin implants can be
placed for deficient chins, while removal of cartilage from the nose may be
necessary to minimize the appearance of the nose. For non surgical options,
the use of fillers – such as Radiesse and Coapetite – may be used to correct

Dr. Lewis Obi was one of the keynote speakers at Palomars Advanced body sculpturing symposium in New York this past weekend. The non FDA approved complex Cytori system is not bad but the cost factor is prohibitive for our domestic patients. Therefore Adivive (Lipokit) is the best and only cost efficient system in the area of stem cell fat."

THE WONDERS OF STEM CELLS
TRADITIONAL FACE LIFT GOES TO THE OPERA
I am amazed but not surprised with my patients response when any mention of fat is suggested as part of their facial procedures: “my friend had Dr. X inject her with fat and none of it took”. The patient pictured above is now two years post Cell Assisted Lipotransfer (CAL) which is the most advanced technique of fat grafting.
Since January of 2011 I have provided Adivive treatments to approximately 200 patients with significant take in 90% of patients treated. More recently the addition of PRP and CAL has not only increased the long term volume retained but has significantly reduced bruising and down time.
Adivive, CAL and PRP is now used in the majority of traditional face lift and eye lid surgery procedures to produce a prolonged natural youthful look and reduce the need for future synthetic fillers.
Typical results after LJO brow lift and eyelid surgery enhanced with adipose derived stem cells (Adivive plus PRP).
In terms of total body aesthetics as well as fee reduction (up to 50%), SlimLipo of the donor site is often performed at the same time.
This patient had her abdomen and face treated on a Wednesday and back to work on Monday.
Very pleased patient who underwent total face lift, LJO brow lift and eyelid surgery and recently had Adivive/PRP to face after weight gain. Post Adivive pictures will be posted after her next visit.
We have been researching the application of adipose derived stem cells in the area of regenerative and therapeutic uses for three years. We are now a participating IRB member of the Cell Surgical Network of California and will extend our technology to other specialties in the near future.

In our March 2012 blog, Dr. Lewis Obi was quoted in Plastic Surgery Practice (PSP) magazine regarding his utilization of the Adivive Fat Processing Unit. The article also featured updates about use of platelets and fibroblasts in cosmetic procedures. To date, Dr. Obi has performed over 100 Adivive autologous fat transfer procedures and since the publication of the PSP article has combined the rejuvenating effects of platelet-rich plasma (PRP).

interviewed as an authority on Adivive Breast Enhancing by Australia’s
leading health and beauty magazine “Cosmetic Surgery and Beauty.” In the
article, written by Jenni Gilbert, Dr. Obi is quoted regarding his use and
success of the Adivive Fat Transfer System to date. Coincidentally, Dr. Obi
acquired the first FDA approved unit in the United States in January of last
year.
